Your Appointment

When you call our office, we will guide you through scheduling your appointment. If you are experiencing pain, let our team know. We will do our best to evaluate you as soon as possible. When you call for your appointment here is a list of questions that we will ask you before your visit:
  • Your name and telephone number
  • Your date of birth
  • Your referring dentist and tooth number or area in question
  • The type of discomfort you are experiencing
  • Do you have any dental insurance?
  • The name of the dental insurance company and subscriber information
We will ask you to arrive 15 minutes ahead of your appointment time to complete your health information so we may be able to seat you as close to your appointed time as possible.
